In the previous two sections, benzoin reactions with aldehydes as acceptor substrates are described. If, instead, a ketone is applied as the acceptor substrate, α-hydroxy ketones are formed that contain a tertiary carbinol moiety. This is also the case with α-keto acids as acceptors resulting in the formation of acetohydroxy acids [2-hydroxy-2-methyl-3-oxobutanoate (acetolactate) is the simplest derivative of this substance class]. These products, however, are labile to some extent and can nonenzymatically decarboxylate resulting in racemic acyloins. Moreover, acetolactate decarboxylases also catalyze such decarboxylations.[‌163‌,‌164‌]
